Financial Assistance and Charity Care
If you are a patient at The Medical Center at Caverna, we have financial assistance programs to help you. If your current income is two times or less of the Federal Poverty Level for your family size, and you have not been able to obtain government assistance, you could be eligible for charity care (care at no cost to you). If your income is more than twice the Federal Poverty Level, you may be eligible for other assistance programs. Please contact our Financial Services department at 270.786.2191 for more information.
To apply for financial assistance, complete the
Application for Financial Assistance form, print and send us proof of your income or eligibility to participate.
Proof Of Income And Eligibility To Participate
In order for us to determine if you qualify for our assistance programs, you will need to send us proof of income for you and each person in your household, or proof of your participation in one of the public assistance programs listed below. If you are attempting to qualify because of your income level, we require proof of your income for the past 3 months (copies of pay stubs, Social Security benefits, alimony checks, etc.), as well as the most recent tax return (including W-2s and all pages of the return). If you or your family participates in any of the following programs, you can send us proof of your participation in the program instead of proof of income:
